PLEASE NOTE:

Not all workshops are approved for CTG

To be eligible for the VCF subsidy, you MUST be a VWO staff who is Singaporean/PR.

(1) Continue with and complete the current online registration. Once completed, you will receive an automated acknowledgement email of your registration.

(2) Submit VCF application through your VWO. Once approved, submit the VCF Approval Email to info@dasacademy.edu.sg and make payment of nett course fee at DAS Academy prior to the start of the course.

PLEASE NOTE:

To be eligible for the Caregivers Training Grant (CTG), you MUST:

(1) Continue with and complete the current online registration.

(2) CTG applicants need to pay co-payment of $10 and be paid via Paypal OR at DAS Academy (REX House)

(3) Care recipient must meet the following requirements :
      (a) Above 65 years of age OR
      (b) Have a disability
      (c) As certified by a Singapore Registered Doctor OR
           i. Doctor’s report/memo
           ii. Functional Assessment Report
           iii. IDAPE/Eldershield approval letter
      (d) Is currently receiving services from a voluntary welfare organization (VWO)

(4) Download the CTG form here or from the 'Workshops' main page. Submit the original completed form (endorsed by the respective Voluntary Welfare Organisation) with all supporting documents by post or in-person to DAS Academy Ltd, 73 Bukit Timah Road, #05-01, S(229832) at least 2 weeks before commencement of the workshop.

(5) Please bring the original CTG form on the day of workshop.

*The co-payment is non-refundable and non-transferable if you are unable to attend or if the CTG application is not approved.

PLEASE NOTE:

1. The $200 credit can be utilised for any of the following DAS Academy online Workshops (via live stream) from 10 April 2020 onwards

2. Registrations are strictly to be made by 31st May 2020 for workshops conducted up to 30th November 2020.

3. Applications are to be made at least two weeks prior to the commencement of each workshop.

4. A non-refundable co-payment of $10 for each workshop will be required upon registration.

5. This offer is not transferable to other households members or redeemable for cash.

6. For non-attendance of a registered workshop, the credits cannot be reused for another workshop

I understand that the use of the S$90 discount voucher is subject to verification. In the event that the voucher cannot be used, I will pay the full amount of the workshop fee. I also understand that the original voucher must be presented to DAS Academy before the commencement of the workshop.

Dear Sir/Madam,

Please upload a scanned copy of official letter from your school/organisation to confirm the sponsorship, and submit the original letter on the start date of training.

Please download the sponsorship form which is to be filled out and attached to the registration form. The original will have to be submitted to DAS Academy on the commencement date of training.
